ABSTRACT

Abstract Aim: This study verified how much performance damage can high-intensity efforts cause to FT shooting efficiency. Methods: A sample of 13 male amateur basketball players (19.9 ± 3.2 years; 76.7 ± 8.7 kg; 182 ± 10 cm) participated in the study on three non-consecutive days. A maximum progressive exercise test determined maximum heart rate (HRMax) during the first session. On the other two days (randomly assigned), athletes performed five bouts of basketball-related exercises, intense enough to raise HR to either 65% (S65) or 90% (S90) of HRMax, followed by two FT shots (totaling 10 FT). Results: FT performance was lower in S90 than in S65 (56.9% ± 18.9% vs 73.1% ± 12.5%, respectively; p = 0.026; ES = 1.01 - "large effect size"). Magnitude-based inference analysis considered shooting at S90 moderately disadvantageous and odds ratio analysis suggest that shooting FT at higher HR values represents an 11-fold chance to worsen performance (OR = 11.1; 95% CI = 1.79 to 68.9; p = 0.01). Conclusion: Basketball FT shooting efficiency is impaired after a bout of game-related high-intensity activity.

ABSTRACT

A análise do desempenho esportivo deve ser feita através de uma perspectiva que contemple a influência de diversos fatores. Neste contexto, fatores socioeconômicos e demográficos do estado apresentam-se, também, como preditores no desempenho esportivo, sobretudo em esportes que aparentemente são considerados "baratos e de fácil acesso" como é o caso da corrida de rua. Sendo assim, o propósito do trabalho é apresentar uma descrição da distribuição dos melhores corredores de rua a nível nacional, e verificar a relação entre a representatividade no ranking e variáveis relacionadas ao estado, sendo elas: índice de desenvolvimento humano, tamanho da população, rendimento mensal e produto interno bruto. Para verificação da distribuição dos corredores no país, foi verificada a frequência de atletas no ranking, conforme estado. A correlação de Sperman foi utilizada para verificar a relação existente entre as variáveis relacionadas ao estado e a presença dos corredores no ranking, sendo considerado p<0,05. Foi observada uma maior representatividade das regiões Sudeste e Sul, comparativamente às demais regiões do Brasil, para além do fato de que houve correlação positiva e significativa entre as variáveis "tamanho da população" e "produto interno bruto" com os estados representados no ranking. Deste modo, conclui-se que àquelas regiões que possuem melhores valores desses indicadores são mais propensas a obterem uma maior quantidade de atletas no ranking por fatores como: maior número de atletas praticantes da modalidade e fatores econômicos que sustentam os seus praticantes durante a dedicação ao esporte e que culminam em uma maior representatividade relativamente às demais regiões. The analysis of sport performance should be performed through different predictors. Taking this into account, socioeconomic and demographic factors from the state where the person lives can be seeing as relevant determinants of the sport performance, especially in sports considered as "cheap and accessible", such as road running race. The purpose of the present study is to describe the distribution of the best road runners in Brazil, and to verify the relationship between the representativeness in the national ranking with variables related to the state, namely: human development index, state population size, monthly income, and gross domestic product. To determine the road runners' distribution across the country, the frequency of the runners presented in the ranking was reported according to Brazilian states. The Sperman correlation (r) was computed to verify the relationship between state predictors and the presence of runners in the ranking, with the significance level of p<0.05. It was observed a higher representativeness of the South and Southeast regions, compared with the other Brazilian geographical regions; in addition, a positive and significant correlation was observed between the variables "state population size" and "gross domestic product" with the states presented in the ranking. In conclusion, those regions with higher values in these indicators are more prone to have more runners in the ranking, given the highest number of subjects enrolled in this sport as well as socioeconomic factors that support the athletes during their practice, leading to a higher representativeness of this regions in relation to the other ones. ABSTRACT Introduction: Among the main challenges faced by coaches of team sports are to create an environment and conditions that will enable players to reach their optimal level of physical performance at the start of the competitive season, and to maintain this level throughout the season. Objective: The purpose of this study was to assess the effects of six weeks of additional training with 3-against-3 (3v3) small-sided games (SSG) on the physical performance of elite nonstarter basketball players. Methods: Eleven professional basketball players (five starters and six nonstarters) performed the same training program throughout the regular season, with the only difference that after each game, the starters (S) performed low intensity shooting drills while the nonstarters (NS) performed half-court 3v3 SSG. At the start and end of the six-week period, the athletes were assessed for muscle power, speed, agility and aerobic power. Two-way analysis of variance and effect sizes were used. Magnitude-based inferential analyses were used to complement the parametric tests. Results: After the six weeks, the S and NS showed improvement (p<0.05) in the squat jump (S=5.8% and NS=7.1%), counter movement jump (S=3.7% and NS=3.6%), 20m-sprint (S=3.4% and NS=2.0%), agility T test (S=3.6% and NS=3.1%) and Yo-yo intermittent recovery (S=5.2% and NS=2.5%). No differences were found between groups and moments. The effect sizes showed moderate effect on agility and little effect on jumping ability, for both groups; moderate, for S, and little, for NS, on speed and aerobic conditioning. Magnitude-based inferences indicated that S were likely to outperform NS in terms of improvement in 20-m sprint ability and possibly also in agility. In regard to the other physical performance variables, the analyses of magnitude-based inferences were inconclusive. Conclusions: The results of this study showed that 3v3 SSG in the short-term on half-court, as an additional training routine for NS, can bring positive results for the physical performance for these players. RESUMEN Introducción: Uno de los principales desafíos de los entrenadores de deportes colectivos es crear ambiente y las condiciones que permitan a los jugadores alcanzar su nivel ideal de desempeño físico al inicio de la temporada competitiva y mantener ese nivel a lo largo de la temporada. Objetivos: El objetivo de este estudio fue evaluar los efectos de seis semanas de entrenamiento adicional con juegos reducidos (JR) 3 contra 3 (3x3) sobre el desempeño físico de jugadores suplentes del baloncesto de élite. Métodos: Once jugadores profesionales (cinco titulares y seis suplentes) realizaron el mismo programa de entrenamiento durante la temporada regular, con la única diferencia de que, después de cada partido, los titulares (T) realizaban ejercicios de lanzamiento de baja intensidad, mientras que los suplentes (S) realizaban JR 3x3 en media cancha. Al inicio y al final de cada período de seis semanas los atletas fueron evaluados en cuanto a la potencia muscular, velocidad, agilidad y potencia aeróbica. Fueron usados análisis de variancia y tamaños de efecto de dos vías. Fueron usados análisis inferenciales basados en magnitud para complementar los tests paramétricos. Resultados: Después de seis semanas, el grupo T y el S mostraron mejora (p <0,05) en el salto con agachamiento (T = 5,8% y S = 7,1%), salto contramovimiento (T = 3,7% y S = 3,6%), sprint de 20 metros (T = 3,4% y S = 2,0%), test T de agilidad (T = 3,6% y S = 3,1%) y el test de recuperación intermitente Yo-yo (T = 5,2% y S = 2,5%). No fueron encontradas diferencias entre grupos y momentos. Los tamaños de efecto mostraron efecto moderado en la agilidad y pequeño efecto en la capacidad de salto para ambos grupos; efecto moderado para el grupo T y pequeño para el S, en la velocidad y acondicionamiento aeróbico. Las inferencias basadas en la magnitud indicaron que el grupo T probablemente superaría al grupo S en términos de mejora de la habilidad de sprint de 20 metros y posiblemente también en agilidad. El análisis de las inferencias basadas en magnitud no fue concluyente con relación a las otras variables. Conclusiones: Los resultados de este estudio mostraron que JR 3x3 en media cancha como entrenamiento adicional para el grupo S puede ser positivo para el desempeño físico de esos jugadores. RESUMO Introdução: Um dos principais desafios dos treinadores de esportes coletivos é criar ambiente e as condições que permitam aos jogadores atingir seu nível ideal de desempenho físico no início da temporada competitiva e manter esse nível ao longo da temporada. Objetivos: O objetivo deste estudo foi avaliar os efeitos de seis semanas de treinamento adicional com jogo reduzido (JR) 3 contra 3 (3x3) sobre o desempenho físico de jogadores reservas do basquetebol de elite. Métodos: Onze jogadores profissionais de basquetebol (cinco titulares e seis reservas) realizaram o mesmo programa de treinamento durante a temporada regular, com a única diferença de que, depois de cada jogo, os titulares (T) realizavam exercícios de arremesso de baixa intensidade, enquanto os reservas (R) realizavam JR 3x3 em meia quadra. No início e no final do período de seis semanas, os atletas foram avaliados quanto à potência muscular, velocidade, agilidade e potência aeróbica. Foram usadas análises de variância e tamanhos de efeito de duas vias. As análises inferenciais baseadas em magnitude foram usadas para complementar os testes paramétricos. Resultados: Depois das seis semanas, o grupo T e o R mostraram melhora (p < 0,05) no salto com agachamento (T = 5,8% e R = 7,1%), salto contramovimento (T = 3,7% e R = 3,6%), sprint de 20 metros (T = 3,4% e R = 2,0%), teste T de agilidade (T = 3,6% e R = 3,1%) e o teste de recuperação intermitente Yo-yo (T = 5,2% e R = 2,5%). Não foram encontradas diferenças entre grupos e momentos. ABSTRACT

O objetivo do presente estudo é descrever a prevalência de deslocamento ativo entre adolescentes brasileiros no trajeto casa-escola considerando as unidades federativas e regiões do país, e verificar os preditores associados a esse comportamento. A amostra foi composta por 99.967 adolescentes (ambos os sexos; média de idade 14,24 anos) que participaram da Pesquisa Nacional de Saúde do Escolar em 2015. Para o estudo, foram consideradas informações relativas ao deslocamento ativo no percurso casa-escola (frequência e duração), aspecto socioeconômico e percepção de segurança no trajeto para a escola, sendo obtidas através de questionário. A análise estatística foi realizada por meio da regressão logística binária. Foi observado que 44,4% dos jovens realizam o deslocamento ativo (ir e voltar da escola através do uso de bicicleta/caminhada, pelo menos cinco dias na semana), sendo que a região Sudeste apresenta a maior porcentagem de adolescentes que aderem à tal prática (46,4%). Ser do sexo masculino (OR = 1,19), mais "novo" (OR = 0,89) e despender mais tempo por semana em deslocamento ativo (OR = 1,01) aumentam as chances de o adolescente realizar o deslocamento ativo em seu trajeto para a escola; enquanto que possuir carro em casa (OR = 0,72) e perceber o trajeto para a escola como inseguro (OR = 0,66) reduzem as chances de adoção a tal comportamento. Osresul-tados reforçam a característica multifatorial do deslocamento ativo, e a necessidade de intervenções centradas no sujeito e no ambiente


The purpose of this study is to describe the prevalence of active commuting among Brazilian adolescents in their route home-school by federal units and country regions, and to determine the predictors related to it. Sample comprised 99,967 adolescents (both sexes, mean age 14.24 years) that had taken part in the Bra-zilian National Adolescent School-based Health Survey in 2015. For the present study, it was considered information related to active commuting home-school (frequency and duration), socioeconomic status, and security perception at home-school route, which were obtained through questionnaire. Statistical analysis was performed by logistic regression. It was observed that 44.4% of adolescents reported to use the active commuting (go to and from school using bicycle or walking for, at least, five days a week), and the Southwest region showed the highest prevalence of adolescents who use the active commuting (46.4%). Boys (OR = 1.19), younger adolescents (OR = 0.89), and those who spent more weekly time in active commuting (OR = 1.01) are more prone to be adopt the active commuting in their route to and from school; on the other hand, having a car at home (OR = 0.72) and perceive the route to/from school as insecure (OR = 0.66) decrease their chances to adopt this behaviour. Results highlight that active commuting is a multifactorial trait, and the relevance to develop intervention strategies that should take into account both, subjects and the environment

ABSTRACT

Objetivos: Investigar o surto de histoplasmose em bombeiros no Distrito federal/DF, ocorrido em junho de 2017. Métodos: Realizou-se um estudo de coorte por meio das entrevistas realizadas com os bombeiros mediante um questionário semiestruturado. Considerou-se infectado o bombeiro que apresentou tomografia de tórax sugestiva de histoplasmose ou reagente nos testes de imunodifusão e/ou Western blot. Coletou-se amostra ambiental e realizou-se Nested PCR específico para Histoplasma capsulatum. Resultados: Entre 35 bombeiros, 94,3% eram homens; com a mediana de idade de 37 (24-45) anos, 28 foram classificados como infectados. A média de permanência dentro da caverna foi 25 minutos. O fator de risco associado à infecção foi o ato de entrar na caverna (RR=3,86; RA=71,6; p<0,02). Entre 14 amostras ambientais, 50% foram positivas para H. capsulatum. Conclusão: Confirmou-se o surto de histoplasmose de bombeiros em Brazlândia-DF, e foram tomadas ações como a interdição da caverna e o tratamento dos bombeiros.


Objectives: To nvestigate the outbreak of histoplasmosis in firefighters in Federal District/DF, which occurred in June 2017. Methods: A cohort study was conducted through interviews with firefighters by means of a semi-structured questionnaire. Firefighters who presented chest tomography suggestive of histoplasmosis or reacted to immunodiffusion and/or Western blot tests were considered infected. Environmental samples were collected and Nested PCR specific for Histoplasma capsulatum was performed. Results: Among 35 firefighters, 94.3% were men; with a median age of 37 (24-45) years, 28 were classified as infected. The average length of stay inside the cave was 25 minutes. The risk factor associated with infection was the act of entering the cave (RR = 3.86, RA = 71.6, p <0.02). Among 14 environmental samples, 50% were positive for H. capsulatum. Conclusion: The outbreak of histoplasmosis in firefighters in Brazlândia-DF was confirmed, and actions were taken such as banning the cave and treating the firefighters

ABSTRACT

Canine demodicosis is a common inflammatory parasitic skin disease caused by Demodex mites. House dust mites, such as Dermatophagoides spp., play an important role in the pathogenesis of canine atopic dermatitis (AD). The goal of this experimental work was to investigate whether demodectic dogs could be previously exposed/sensitized to house dust mites' antigens. First the prevalence of demodicosis in a southeastern region of Brazil was investigated by analyzing clinical files of dogs that were admitted to a Veterinary Hospital. Subsequently, the IgG responses to Dermatophagoides pteronyssinus (Dp) and Dermatophagoides farinae (Df) and IgE to D.pteronyssinus (Dp) were evaluatedin two groups, AD or demodicosis dogs. Additionally, the major IgE-binding Dp proteins that are recognized by sera from dogs with demodicosis and AD were evaluated. A total of 2,599 clinical files were analyzed to identify the major parasitic skin diseases in dogs from this region, considering the age, sex and breed of the animals. The epidemiological study identified 111 animals with skin diseases; from these 20.7% presented demodicosis. Afterwards, serum samples were obtained from another groups of demodicosis, AD, and healthy dogs, and analyzed for Dp and Df-specific IgG, and IgE antibody levels, Dp IgG avidity by ELISA and IgE-binding Dp-specific proteins by immunoblot. IgG and IgE antibodies to Dp were detected in sera from additional groups of dogs with AD, demodicosis or healthy, with higher IgE levels to Dp in AD than demodectic or healthy dogs. IgG to Df was detected, despite with smaller levels compared to Dp in sera from demodectic dogs, and also in healthy dogs. Immunoblot showed IgE-binding to Dp proteins in sera of dogs with demodicosis and AD; with strong reactivity for the 72 and 116 kDa antigens detected by sera from demodicosis dogs. However, sera from healthy dogs >12 months old also presented reactivity to these bands. In conclusion, the detection of Dp-IgG and IgE antibodies in sera from demodectic dogs indicates previous exposure and sensitization to the house dust mite, respectively, more than cross-reactivity between demodex mites and Dp antigens detected by canine antibodies. Additionally, higher Dp-specific IgE levels were found in dogs with AD compared with those with demodicosis or healthy, suggesting that Dp-specific IgE could better discriminate dogs with AD from healthy ones or even those with demodicosis.


Demodicose canina é uma doença inflamatória comum da pele causada por ácaros do gênero Demodex. Ácaros da poeira doméstica como Dermatophagoides spp. desempenham papel importante na patogênese da dermatite atópica canina (DA). O objetivo desse trabalho experimental foi investigar se cães com demodicose poderiam ser previamente expostos/sensibilizados com antígenos de ácaros da poeira doméstica. A princípio, investigou-se a prevalência de demodicose em uma região sudeste do Brasil, analisando-se prontuários clínicos de cães admitidos em um Hospital Veterinário. Posteriormente, as respostas de IgG a Dermatophagoides pteronyssinus (Dp) e D. farinae (Df) e IgE a D. pteronyssinus (Dp) foram avaliadas em dois grupos, DA ou demodicose. Também foram avaliadas as principais proteínas Dp reconhecidas por anticorpo IgE presente em soros de cães com demodicose e DA. Um total de 2.599 prontuários clínicos foram analisados para identificar as principais doenças parasitárias da pele em cães dessa região, considerando a idade, sexo e raça dos animais. O estudo epidemiológico detectou 111 animais com doenças de pele e destes, 20,7% apresentavam demodicose. Posteriormente, amostras de soro foram obtidas de outros grupos de cães com demodicose, DA ou saudáveis, e analisadas quanto aos níveis de IgG e IgE específicos para Dp e Df, avidez de IgG a Dp por ELISA e proteínas específicas de Dp reconhecidas por IgE por immunoblot. Anticorpos IgG e IgE para Dp foram detectados em soros de grupos adicionais de cães com DA, demodicose ou saudáveis, com níveis mais altos de IgE para Dp na DA do que no soro de animais saudáveis. Níveis de IgG específicos para Df foram detectados, apesar serem menores em comparação com os detectados para Dp em soros de cães demodéticos, e também em cães saudáveis. A análise de immunoblot demonstrou detecção de IgE para proteinas de Dp em soros de cães com demodicose e DA; com forte reatividade para os antígenos de 72 e 116 kDa detectados por soros de cães com demodicose. No entanto, soros de cães saudáveis > 12 meses de idade também apresentaram reatividade a essas bandas. Em conclusão, a detecção de anticorpos Dp-IgG e IgE específicos em soros de cães demodéticos indica exposição prévia e sensibilização aos ácaros, respectivamente, mais do que reatividade cruzada entre ácaros Demodex e antígenos Dp detectados por anticorpos caninos. Além disso, níveis de Dp-IgE específicos mais elevados encontrados em cães com DA, sugerem que esses anticorpos poderiam discriminar melhor cães com DA daqueles saudáveis ou mesmo demodéticos.

ABSTRACT

Abstract The present report describes the first case of postpartum disseminated histoplasmosis in a 24-year-old HIV-negative woman. On the tenth day after vaginal delivery, the patient presented with dyspnea, fever, hypotension, tachycardia, and painful hepatomegaly. Yeast-like Histoplasma capsulatum features were isolated in the buffy coat. The phylogenetic analysis demonstrated that the fungal isolate was similar to other H. capsulatum isolates identified in HIV patients from Ceará and Latin America. Thus, histoplasmosis development in individuals with transitory immunosuppression or during the period of immunological recovery should be carefully examined.

Abstract Aims: to identify and describe the clustering of characteristics related to running amongst Brazilian runners using latent class analysis and to verify if there is a profile associated with better performance. Material and Methods: a sample of 1149 Brazilian runners answered an online questionnaire, that provided information about biological (sex, age, height, weight), training (running pace, frequency and volume/week, motivation), and socioeconomic factors, as well as a multidimensional questionnaire of fear of failure. Latent Class Analysis was used to identify subgroups of Brazilian runners, based on BMI, training volume and frequency/week, motivation, socioeconomic factors, and the fear of failure. Further, a χ2 test was computed to verify statistical differences in the frequency of the descriptive variables between classes. Finally, binary logistic regression analysis estimated factors associated with running performance, with running pace as the dependent variable. Results: It was possible to identify two different classes among Brazilian runners, which were called "amateur runners" and "recreational runners". Variables that highlighted classes' differences were: volume and frequency training/week, motivation for the practice, and BMI. Regarding the running performance, logistic regression analysis showed that men (OR=5.39; 95%CI=4.00-7.25), young runners (OR=0.38; 95%CI=0.28-0.51), and "amateur runners" (OR=4.19; 95%CI=2.95-5.94) were more prone to have higher performance. Conclusion: Two distinct classes were found among Brazilian runners that were linked to their performance, highlighting that even among non-professional runners, differences can be observed regarding their classification. Hence, future studies should consider using these classes to properly stratify or identify non-professional runners.

ABSTRACT

The use of technology has increased tremendously, by means of more reliable, smaller, more accessible and specially more user-friendly devices, which provide a wider range of features, and promote significant benefits for the population and health professionals. It is in this context that monitors and apps for heart rate (HR) measurement have emerged. HR is a clinical vital sign of diagnostic and prognostic importance. In response to body movement, HR tends to increase, in a direct relationship with the intensity of exercise. HR was primarily measured by the count of arterial pulse, and recently, HR can be precisely measured by monitors, bracelets and smartphone apps capable to perform real-time measurements and storage of data. This paper aimed to make a brief and updated review on the theme, providing a broader view of advantages and limitations of these resources for HR measurement in exercise. HR monitors and apps use basically two types of technology, optical sensor (photoplethysmography) and electrical signal from the heart. In general, these devices have shown good accuracy in measuring HR and HR variability at rest, but there are differences between brands and models considering the type, mode and intensity of exercise. HR measurements by monitors and smartphone apps are simple, accessible and may help cardiologists in the monitoring of the intensity of aerobic exercise, focusing on health promotion and on primary and secondary prevention of cardiovascular diseases

ABSTRACT

O objetivo deste estudo é investigar as representações sociais de agricultores rurais do povoado Várzea dos Cágados na Cidade de Lagarto, Sergipe, Brasil, sobre as queixas de saúde decorrentes da exposição a substâncias ototóxicas e o trabalho do fonoaudiólogo na saúde desses trabalhadores. Trata-se de uma pesquisa exploratória com abordagem qualitativa e quantitativa. Definiu-se a estratégia da entrevista semiestruturada para a coleta de dados. A abordagem do Discurso do Sujeito Coletivo foi aplicada na avaliação dos dados resultantes das entrevistas. Os resultados mostraram que 62,96% dos participantes da pesquisa possuíam o ensino fundamental incompleto; 59,26% contavam com mais de 31 anos de atuação como agricultores; todos os agricultores utilizavam equipamentos de proteção individual em suas atividades laborais; 48,15% dos agricultores possuía renda acima de 1 salário mínimo. Os discursos dos agricultores irrigantes contemplaram aspectos referentes aos efeitos do uso do agrotóxico, prevenção de acidentes e equipe de segurança no trabalho. Concluiu-se que os agricultores rurais participantes da pesquisa não expressaram nenhuma representação sobre os profissionais da saúde do município como membros de campanhas ou atividades que buscassem prevenir e promover a saúde deles


The objective of this study is to investigate the social representations of rural farmers in Várzea dos Cágados village in the city of Lagarto, Sergipe, Brazil, on health complaints due to exposure to ototoxic substances and speech therapist work for the health of these farmers. This is an exploratory research with a qualitative and quantitative approach. The semi-structured interview was defined as strategy for data collection. The data resulting from the interviews were evaluated, considering the Collective Subject Discourse approach. The results showed that 62,96% of the participants had incomplete primary education; 59,26% had more than 31 years working as farmers; all farmers used personal protective equipment in their work activities; 48,15% of farmers had income above minimum wage. The discourses of the irrigation farmers included aspects related to the effects of the use of pesticides, accident prevention and safety team at work. It was possible to conclude that the rural farmers participating in the research did not express any representation about the health professionals of the municipality as members of campaigns or activities that sought to prevent and promote their health.


El objetivo de este estudio es investigar la representación de los agricutores del pueblo de Várzea dos Cágados en la ciudad de Lagarto, Sergipe, Brasil, sobre las quejas de salud por la exposición a substancias ototóxicas así como el trabajo del fonoaudiologo en la salud de estos trabajadores. Tratase de una investigación exploratoria con enfoque cualitativo y cuantitativo. Fue definida la estrategia de la entrevista semiestructurada para recolección de datos. Los datos resultantes de las entrevistas fueron evaluados, considerando el abordaje del discurso del sujeto colectivo. Los resultados mostraron que el 62,96% de los participantes en la investigación tenía el conocimiento básico incompleto, 52,26% contaban con más de 31 años ejerciendo la agricultura; todos utilizaban equipos de protección individual en sus actividades laborales; el 48,15% tiene ingresos superiores a 1 salario mínimo. Los discursos de los agricultores irrigadores contemplaron aspectos referentes a los efectos del uso de pesticidas, prevención de accidentes y del equipo de seguridad en el trabajo. Concluyóse que los agricultores rurales que participaron en la investigación no expresaron participación alguna de los profesionales en salud del municipio, como miembros en campañas o actividades que para prevenir y promover la salud.

ABSTRACT

BACKGROUND Paracoccidioidomycosis (PCM) is one of the most important systemic mycoses in Latin America and the leading fungal cause of mortality in non-immunosuppressed individuals in Brazil. However, HIV/PCM co-infection can increase the clinical severity in these co-infected patients. This co-infection is rarely reported in the literature mainly because of the different epidemiological profiles of these infections. Furthermore, PCM is a neglected and non-notifiable disease, which may underestimate the real importance of this disease. The advent of molecular studies on the species of the genus Paracoccidioides has expanded the knowledge regarding the severity and the clinical spectrum in PCM. In this context, the development of studies to describe the association of the Paracoccidioides phylogenetic cryptic species in vulnerable populations, such as HIV-infected patients, appears relevant. OBJECTIVE To describe the clinical, epidemiological, therapeutic and prognostic aspects in HIV/PCM co-infected patients, along with the molecular identification of the Paracoccidioides species involved in these cases. METHODS The investigators performed a molecular and clinical retrospective study involving HIV/PCM co-infected patients, from a reference centre for PCM care in the endemic area of Rio de Janeiro, Brazil, from 1998 to 2015. Molecular identification of the fungal strains was done by amplification of partial sequences of arf and gp43 genes. FINDINGS Of 89 patients diagnosed with PCM by fungal isolation in the culture, a viable isolate was recovered for molecular analysis from 44 patients. Of these 44 patients, 28 (63.6%) had their serum samples submitted for enzyme immunoassay tests for screening of HIV antibodies, and 5 (17.9%) had a positive result. All cases were considered severe, with a variable clinical presentation, including mixed, acute/subacute clinical forms and a high rate of complications, requiring combination therapy. Paracoccidioides brasiliensis S1 was the species identified in all cases. CONCLUSIONS HIV/PCM co-infection can change the natural history of this fungal disease. The authors reinforce the need to include HIV screening diagnostic tests routinely for patients with PCM.

ABSTRACT

Abstract The authors report the first case of fatal septic shock, a rare clinical presentation of paracoccidioidomycosis (PCM) caused by Paracoccidioides brasiliensis S1. We also provide an immunological evaluation of the patient. Severe clinical signs such as organ dysfunction and digital gangrene occurred in this case. The patient presented a remarkable cell activation profile and diminished percentage of peripheral blood T regulatory cells. A decrease in anti-inflammatory IL-1RA plasma level showed the potential for endothelium damage, probably contributing to a vasculitis process. Together with P. lutzii, P. brasiliensis appears to be involved in severe cases of PCM.

BACKGROUND Human trichinellosis is a foodborne parasitic zoonotic disease caused by ingestion of raw or undercooked meat infected with nematode larvae of the genus Trichinella. In the USA, sporadic cases and outbreaks caused by the consumption of wild game meat infected with Trichinella have been reported. The current methods for diagnosis such as serology and microscopy are not specific, may result in false negative results, and cannot differentiate encapsulated Trichinella larvae to species level. The molecular protocols currently available for the differentiation of all encapsulate Trichinella species prevalent in North America have some limitations such as the inability to identify and resolve the presence of several Trichinella species in a single test. OBJECTIVES/METHODS In this study we developed and evaluated a multiplex TaqMan quantitative real-time polymerase chain reaction (qPCR) assay, which can simultaneously detect, identify and differentiate all species of encapsulated Trichinella occurring in North America i.e., T. nativa, T. spiralis, T. murrelli and Trichinella T6, even in cases of multiple infection in a single sample. We investigated two human biopsies and 35 wild animal meat samples considered as having a high likelihood of harboring Trichinella larvae obtained from the United States during 2009-2017. FINDINGS Using the multiplex assay describe here, 22 (59%) samples that tested positive contained Trichinella spp., were identified as: T. nativa (n = 7, including a human biopsy), T. spiralis (n = 9, including a human biopsy), T. murrelli (n = 3), Trichinella T6 (n = 1). Results also included two rare mixed infection cases in bears, a T. nativa/T. spiralis from Alaska and a T. spiralis/Trichinella T6 from California. The species identifications were confirmed using a conventional PCR targeting the rRNA ITS1-ITS2 region, followed by DNA sequencing analysis. The estimated limit of detection (LOD) was approximately seven larvae per gram of meat. MAIN CONCLUSIONS Differentiation of Trichinella spp. is needed to improve efforts on identification of case, optimize food safety control and better understand the geographic distribution of Trichinella species. The Trichinella qPCR multiplex proved to be a robust, easy to perform assay and is presented as an improved technique for identification of all known encapsulated species occurring in North America continent.

Abstract The aim of this study was to analyze the effectiveness of long distance passes performed during the 2014 Brazil FIFA World Cup. All 64 matches were analyzed, however, actions performed during overtime were not included in the sample. The study defined long pass as an action made by a player from the defensive midfield aiming at passing the ball to another teammate located on the offensive field. The action result assessment followed these criteria: shots on goal, ball possession maintenance, loss of ball possession and ball possession recovery. Total long distance passes were also considered in the analysis. Throughout the tournament, 4,512 long distance passes were attempted. The main findings were that 59% resulted on loss, 28% on maintenance and 12% on recovery of the ball possession, but only less than 1% resulted in shots on goal (F = 505.5; p<0.001; partial ƞ2 = 0.76). Teams with the highest number of long pass attempts loose ball possession more frequently. There were more long distance passes at the first and at the last 15 min of matches. UEFA and Concacaf teams executed, respectively, the lowest and the highest number of long passes. The major outcome of this study is that long distance passes have low effectiveness due to the high rate of loss of ball possession, rarely creating score opportunities. The more the teams executed long passes, the more the teams lose ball possession.


Resumo Objetivou-se analisar a eficácia dos passes de longa distância realizados durante os jogos da Copa do Mundo da FIFA 2014. Todas as 64 partidas foram analisadas. Porém, as prorrogações na fase eliminatória da competição não foram acrescidas na amostra. O estudo definiu o lançamento como uma ação feita por um jogador do meio campo defensivo com o objetivo de passar a bola para outro colega de equipe localizado no campo ofensivo. O resultado da ação foi avaliado com base nos seguintes critérios: finalização ao gol, manutenção da posse de bola, perda da posse de bola e recuperação da posse de bola. O total dos lançamentos também foi considerado nas análises. Ao longo do torneio houve 4.512 lançamentos. Os principais achados foram que 59% dos lançamentos resultaram em perda, 28% em manutenção e 12% em recuperação da posse da bola, mas apenas menos de 1% resultaram em finalização ao gol (F = 505,5; p <0,001 Parcial ƞ2 = 0,76). Equipes que lançaram mais perderam a bola com maior frequência. Houve mais lançamentos nos primeiros e nos últimos 15 min de jogo. Equipes da UEFA e da Concacaf executaram, respectivamente, o mais baixo e o mais alto número de lançamentos. O principal achado deste estudo é que os lançamentos apresentam baixa eficácia devido à alta taxa de perda da posse de bola, raramente criando oportunidades de finalização à meta. Quanto mais lançamentos feitos, mais posse de bola perdida.

Abstract Introduction: The implantable cardioverter defibrillator had been increasing the survival of patients at high risk for sudden cardiac death. The subcutaneous implantable cardioverter defibrillator was developed to mitigate the complications inherent to lead placement into cardiovascular system. Objective: To report the initial experience of 18 consecutive cases of subcutaneous implantable cardioverter defibrillator implantation showing the indications, potential pitfalls and perioperative complications. Methods: Between September 2016 and March 2017, 18 patients with indication for primary and secondary prevention of sudden cardiac death, with no concomitant indication for artificial cardiac pacing, were included. Results: The implantation of the subcutaneous implantable cardioverter defibrillator successfully performed in 18 patients. It was difficult to place the subcutaneous lead at the parasternal line in two patients. One patient returned a week after the procedure complaining about an increase in pain intensity at pulse generator pocket site, which was associated with edema, temperature rising and hyperemia. Two patients took antialgic medication for five days after surgery. A reintervention was necessary in one patient to replace the lead in order to correct inappropriate shocks caused by myopotential oversensing. Conclusion: In our initial experience, although the subcutaneous implantable cardioverter defibrillator implantation is a less-invasive, simple-accomplishment procedure, it resulted in a bloodier surgery perhaps requiring an operative care different from the conventional. Inappropriate shock by oversensing is a reality in this system, which should be overcame in order not to become a limiting issue for its indication.

ABSTRACT

O objetivo desta pesquisa foi analisar a formação de treinadores de handebol de âmbito escolar, bem como identificar os contextos e situações de aprendizagem preferidos por eles. Para isso foram entrevistados 11 treinadores de âmbito escolar de um município do Estado de São Paulo. O instrumento de entrevista semiestruturada abrangeu duas questões principais: uma relacionada à formação acadêmica (graduação e pós-graduação) e outra referente aos diferentes contextos de aprendizagem. Os entrevistados relataram que a graduação cumpre um importante papel na formação, porém insuficiente para o domínio dos conteúdos específicos inerentes ao seu contexto de atuação. Considerando os contextos de aprendizagem os treinadores apontam preferência pelas fontes provenientes do contexto informal (como as experiências como atleta, as conversas com outros treinadores e os materiais disponíveis na internet), ao passo que apontam o contexto não formal como pouco relevante para a atuação


This work analyzes school handball coaches' academic background and identifies their preferred learning contexts and situations. Eleven school coaches from a city in the Brazilian State of São Paulo were interviewed. The semi-structured interview included two main questions: one related to college education (undergraduate and graduate studies) and another one related to different learning contexts. Interviewees reported that undergraduate students play an important role in their training, but were not enough to master the specific knowledge related to their work context. Considering the learning contexts, coaches preferred sources coming from informal contexts ­ such as their experiences as athletes, conversations with other coaches, and material available online. Coaches also pointed that non-formal contexts were not very relevant to their work


El objetivo de esta investigación fue analizar la formación de entrenadores escolares de balonmano en el ámbito escolar e identificar los contextos y situaciones de aprendizaje preferidos. Para ello fueron entrevistados 11 entrenadores de ámbito escolar de un municipio de la ciudad de São Paulo. El instrumento de entrevista semiestructurada cubrió dos cuestiones principales: la formación académica (grado y postgrado) y los diferentes contextos de aprendizaje. Los entrevistados relataron que la graduación juega un papel importante en la formación, aunque insuficiente para el dominio de los contenidos específicos de su contexto profesional. Teniendo en cuenta los contextos de aprendizaje, los entrenadores manifiestan preferencia por fuentes provenientes del contexto informal (por ejemplo, la experiencia como atleta, conversaciones con otros entrenadores y los materiales disponibles en internet), mientras que señalan el contexto no formal como poco relevante para la actuación profesional

Abstract The aim of this research was to evaluate the performance and tactical behavior of youth soccer players of teams from base categories. The sample consisted of 152 male soccer players aged 11-17 years who participated in the in the U-13 (n = 50), U-15 (n = 62) and U-17 (n = 40) Sergipe State Championship in the year 2015. The System of Tactical Assessment in Soccer (FUT-SAT) was used to identify the players’ tactical behavior based on the fundamental tactical principles. The main results of the study show that players presented higher tactical offensive performance (50.8 ± 9.8 points) compared to defensive performance (34.3 ± 5.7 points) (p < 0.001; d = 2,1), and the Depth Mobility principle presented the highest performance indices in the offensive phase (64.1 ± 8.7; F = 54.4; p < 0.001; partial ƞ2 = 0.26). Defensive Coverage showed the highest performance indexes in the defensive phase (46.1 ± 24.3; F = 54.4; p < 0.001; partial ƞ2 = 0.26).


Resumo O objetivo da pesquisa foi avaliar o desempenho e o comportamento tático de jovens jogadores de futebol de equipes das categorias de base. A amostra foi composta por 152 jogadores de futebol, do sexo masculino, entre 11 e 17 anos de idade, que participaram do Campeonato Sergipano nas categorias sub-13 (n = 50), sub-15 (n = 62) e sub-17 (n = 40) no ano de 2015. Foi utilizado o Sistema de Avaliação Tática no Futebol (FUT-SAT) para identificar o comportamento tático dos jogadores a partir dos princípios táticos fundamentais do futebol. Os principais resultados do estudo mostraram que os jogadores apresentaram maior performance tática ofensiva (50,8 ± 9,8 pontos) do que a defensiva (34,3 ± 5,7 pontos) (p < 0,001; d = 2,1), sendo que o princípio da Mobilidade foi o que apresentou os maiores índices de performance na fase ofensiva (64,1 ± 8,7; F = 54,4; p < 0,001; ƞ2 parcial = 0,26). A Cobertura Defensiva foi o princípio que apresentou os maiores índices de performance na fase defensiva (46,1 ± 24,3; F = 54,4; p < 0,001; ƞ2 parcial = 0,26).

AIMS This study aimed to verify th erelation ship between of anthropometric and physical performance variables with game-related statistics in professional elite basketball players during a competition. METHODS Eleven male basketball players were evaluated during 10 weeks in two distinct moments (regular season and playoffs). Overall, 11 variables of physical fitness and 13 variables of game-related statistics were analysed. RESULTS The following significant Pearson's correlations were found in regular season: percentage of fat mass with assists (r = -0.62) and steals (r = -0.63); height (r = 0.68), lean mass (r = 0.64), and maximum strength (r = 0.67) with blocks; squat jump with steals (r = 0.63); and time in the T-test with success ful two-point field-goals (r = -0.65), success ful free-throws (r = -0.61), and steals (r = -0.62). However, in playoffs, only stature and lean mass maintained these correlations (p ≤ 0.05). CONCLUSIONS The anthropometric and physical characteristics of the players showed few correlations with the game-related statistics in regular season, and these correlations are even lower in the playoff games of a professional elite Champion ship, wherefore, not being good predictors of technical performance.(AU)
